A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF ALLEN, COLLIN
COUNTY, TEXAS, HEREINAFTER REFERRED TO AS "APPLICANT,"
DESIGNATING CERTAIN OFFICIALS AS BEING RESPONSIBLE FOR, ACTING
FOR, AND ON BEHALF OF THE "APPLICANT" IN DEALING WITH THE TEXAS
DEPARTMENT OF HEALTH, HEREINAFTER REFERRED TO AS
"DEPARTMENT," FOR THE PURPOSE OF PARTICIPATING IN THE
CARDIOVASCULAR HEALTH & WELLNESS PROGRAM FOR COMMUNPPY
ARTHRITIS SERVICE PROJECTS GRANT PROGRAM, HEREINAFTER
REFERRED TO AS THE "PROGRAM"; CERTIFYING THAT THE "APPLICANT'
IS ELIGIBLE TO RECEIVE PROGRAM ASSISTANCE; AND PROVIDING FOR AN
EFFECTIVE DATE.
WHEREAS, funds have been provided as part of a federal grant from the Centers for Disease Control and
Prevention through the Texas Department of Health Arthritis Program for the purpose of allowing the
political subdivisions of the State of Texas to participate in the Program; and,
WHEREAS, the "Applicant" is fully eligible to receive assistance under the "program"; and,
WHEREAS, the "Applicant" is desirous of authorizing an official to represent and as for the "Applicant" in
dealing with the "Department" concerning the "Program"
' NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF ALLEN,
COLLIN COUNTY, TEXAS, THAT:
SECTION 1. The "Applicant" hereby certifies that it is eligible to receive assistance under the "Program."
SECTION 2. The "Applicant" hereby authorizes and directs its City Manager to act for the "Applicant" in
dealing with the "Department" for the purpose of the "Program," and Peter H. Vargas is hereby officially
designated as the representative in this regard.
SECTION 3. This Resolution shall become effective immediately from and after its passage.
DULY PASSED AND APPROVED BY THE CITY COUNCIL OF THE CITY OF ALLEN, COLLIN
COUNTY, TEXAS, ON THIS THE 13r DAY OF APRIL. 2004.
